We created this page to help you prepare for your first visit. You will find all the documents you need to complete before your first visit, directions to our office and answers to some frequently asked questions.

Step 1: Paperwork

Before your visit for the first time, please click on the links below to download the new patient forms. Then print the forms, fill them in completely, and bring them with you to your first appointment. This is very important to our office. We respect your time and do everything in our power to see our patients on time. Completing paperwork before your visit is a vital component to this process. By completing this paperwork ahead of time, you help us see you on time and allow other patients to be seen on time, as well. Cancellation / No-Show Policy: We understand that things happen in life that may force you to change your plans. If an emergency arises, we request that you contact our office at least 24 hours before your scheduled appointment. If you do not show up for your appointment or call less than 24 hours before your appointment, you may be charged a cancellation/no-show fee.

QUESTION: What can I expect on my first visit?
ANSWER:

  • On your first visit, you will meet with Donna for your initial exam, cancer screening and X-rays to determine what type of cleaning you will need. This visit is all about getting to know you and your mouth. Approximate time – 1 hour
  • Your next visit will include your cleaning and meeting Dr. Mike. He will provide a full exam and consult, which includes a video tour of your mouth, going tooth by tooth and a full treatment plan. We prioritize the treatment plan to work with your total body and dental health goals and any financial arrangements you may need. Approxmiate time – 1 hour.
  • We will then start your customized treatment plan according to your needs.

QUESTION: Do You Accept Insurance?
ANSWER: We accept the following dental insurance: Delta Dental, Horizon Blue Cross Blue Shield, Fidelio. If you have dental insurance, we are happy to assist you in obtaining maximum dental insurance benefits. Our office staff will assist you in verifying the coverage that your particular program provides. We accept payment from your insurance carrier for the portion covered. On the date of your office visit, you are responsible for your deductible and the portion we estimate the insurance payer will not cover. If your insurance company payment is not received within 30 days after the date of service, the entire balance is due from you and reimbursement can be obtained directly from your insurance company.

QUESTION: What Forms Of Payment Do You Accept?
ANSWER: Our payment options are as follows. We accept cash, debit card, checks, MasterCard, VISA and Discover. We also offer immediate application processing for Care Credit which provides low monthly payments with an interest free option.

QUESTION: Are There Any Hidden Costs?
ANSWER: No. We do our best to outline all costs up front. The cost for your first is discussed when you call to make an appointment. If more treatment is necessary, we will discuss the cost and procedures before we proceed.

QUESTION: Does Dr. Mike Offer A Free Dental Consultation?
ANSWER: Yes. Dr. Mike offers a free 1/2 hour dental consultation to all new patients. The consultation gives you the opportunity to meet Dr. Mike, our team and see the practice. During the free consultation Dr. Mike will use an intraoral camera to provide patients with an ideal visual communication for a diagnosis and treatment options. Dr. Mike will discuss possible ways to improve your smile, provide treatment options and quotes.
